Took me a while to figure, but the movie is Student Who Wears a Garter Belt, the code being SDMT-554.

Everything is gone for this 2011 DVD, at least from my looking-around.

You can investigate this possibility (I hate the terrible filehosters), but chances are there will be no luck:
