It seems you have all information you need there. Her name is Hanna and she is a model in a website called nujolie.com.

Using Google like this (without quotes) "site:www.nujolie.com hanna" returns a few pages including the following:

http://nujolie.com/index.php?p=2&setid=196 (Set 01)
http://www.nujolie.com/index.php?p=2&setid=535 (Set 05)

I believe browsing the website's gallery, you will find the Set 04 which is cited in that image.
