Seems it's from the Human salvation project, a hentai spin-off of Neon Genesis Evangelion.
A lot of conjecture who made it, maybe even by some of the animators who worked on EVA down to the voice actors. Most people on reddit and other forums seem appalled by it and i am gonna assume Gainax (the studio of EVA) tried to purge it. Since it's from around or before 2000 it doesn't seem readily available online, you'd have to do some digging to find it. At least a google search is revealing more threads and dismay about it's existence than anything else :D.
