Her name is Asami aka Asami Suicide, Asami Sg, Asami Hime.

The picture was taken by a photographer from California who goes by the professional name Ka Ho aka "KaHoDesign" - the watermark "KXPissue" in the lower left corner of the pic belongs to him.

According to his profile on ModelMayhem, the last time he worked with Asami was in 2016 - so this picture was taken at least in 2016 or prior. She is featured in an e-(photo)book titled KXPissue Vol. 4, released in 2017, that you can purchase here:
