Her name is Kelly Erickson aka Kelly Erikson - at least according to the search results provided by pimeyes (restricted version).

Unfortunately, I can't find the origin of OPs pic and better proof. So far, I only found a set of more pics in this old forum post from 2015 here:

I put all of these pics as copies on imagebam - but I can't figure out where it was published.

I do have an assumption, though: In 2006 and 2007, she launched 2 websites (ClubHardcoreTV and ClubKellyErikson) with tons of videos and photosets - you can read about them in these "industry blogs":
